About The Position

Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. Requirements

  • Associate’s degree in nursing
  • 1 year of relevant experience preferably in Hospice
  • Michigan Registered Nurse (RN) license upon hire

Responsibilities

  • Assessing, planning, implementing, coordinating, monitoring and evaluating the strategies and services required to meet the patients/family's end-of-life care needs.
  • Creating and continually adjusting an outcome-based plan of care through utilization of the nursing process, relationship centered care principles, and holistic philosophy, assuring that the changing needs of the patient/family are met while facilitating a pain-free, peaceful, sacred death.
  • Exhibit “Patients and Families First” attitude.
  • Responds with compassion, flexibility and urgency to each patient’s needs.
  • Provides the care that patients want when they want it.
  • Develop a comprehensive plan of care that addresses physical, emotional, cultural and spiritual needs of the patient/family with input from the patient/family and the interdisciplinary team.
  • Evaluate this plan and makes interventions as necessary.
  • Advocate for pain/symptom management.
  • Educate and provide the support to patient/family regarding care issues.
  • Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service.
  • Adheres to the organization’s Mission, Service Standards and Code of Conduct.
  • Direct and supervise LPN and CENA.
  • Administer and document medications/treatments as ordered by the physician according to policies in an accurate and timely manner.
  • Identify and respond appropriately to emergency situations.
  • Attend shift reporting and IDT meetings as necessary to facilitate communication of decisions related to the patients care.
  • Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
  • Performs other duties as assigned.
