About The Position

Logan Health Hospice is seeking an independent, self-directed RN with strong clinical nursing skills to join their compassionate Interdisciplinary Team. This role focuses on providing quality, compassionate care to patients at the end of life, building relationships with patients and families, and guiding them through symptom management, education, and comfort care. The position involves managing a patient caseload, traveling throughout the Flathead Valley, and collaborating with a diverse team to create customized care plans. It offers a unique opportunity to practice nursing at the bedside, focusing on patient comfort and family support.

Requirements

• Current Montana RN license or a multi-state compact license with authorization to practice nursing within the state of Montana required.
• Current Driver’s License required.
• Driving record for the previous thirty-six (36) months must not reflect any major driving offenses (i.e; DUI, convicted of a felony involving a motor vehicle, suspended license, three (3) or more speeding tickets or other moving violations, etc.).
• State law minimum liability coverage limits required for personal vehicles used for LH business.
• Possess knowledge of professional nursing theory, medical terminology, and principles of professional nursing practice.
• RQI Provider (BLS) required within 7 days of hire/transfer or before independent patient care, whichever occurs first.
• Understanding of confidentiality laws governing the provider-patient relationship, including HIPAA guidelines.
• Knowledge of equipment, supplies, and instruments used in a medical office, simple routine clinical laboratory methods, universal blood and body fluid precautions.
• Knowledge of OSHA rules and regulations, established protocol for storing poisons, narcotics, acids, caustics, and flammable items; restrictions imposed by various manage care carriers, various forms inherent to profession.

• Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

• Possess and maintain computer skills including working knowledge of Microsoft Office Suite and ability to learn other software as needed.
• Must be comfortable driving in winter weather and have a reliable vehicle for transportation.

Nice To Haves

• BSN degree preferred.
• Minimum of two (2) years’ experience in a similar position within a medical setting (e.g., physician office, clinic, or hospital) preferred.
• Experience in assigned area(s) preferred.

Responsibilities

• Manage own patient caseload and travel throughout the Flathead Valley visiting patients where they call home.
• Serve as the primary nursing contact for patients, coordinating care from admission through end of life.
• Conduct in-home visits for assessment, symptom management, and comfort-focused interventions.
• Educate patients, families, and caregivers on what to expect and how to provide care and symptom management.
• Collaborate with the interdisciplinary team (physician, social workers, chaplains, CNAs, LPNs, and volunteers) to create a customized patient plan of care.
• Attend regularly scheduled Interdisciplinary Team (IDT) meetings.
• Provide emotional support and a steady presence for families navigating anticipatory grief.
• Participate in on-call shift rotation and perform ad-hoc patient visits as needed.
• Document visits and coordinate medications, equipment, and supplies to ensure seamless care.
• Develop, implement, and coordinate individualized, goal-directed nursing care using the nursing process.
• Maintain nursing standards of care and respond to changing workloads and patient conditions by remaining calm, making objective assessments, prioritizing, and asking for assistance.
• Transcribe and implement provider orders and treatments accurately and efficiently.
• Practice infection control procedures per policy and procedure.
• Identify, verbally evaluate, and report identified reaction changes in the physical and mental conditions of patients.
• Document all encounters with patients seeking health advice according to organizational standards.
• Provide individual instruction and education to patients and families concerning health and medical needs.
• Assess and anticipate patient needs, and coordinate necessary interventions.
• Maintain regular and consistent attendance as scheduled.
