About The Position

This position is accountable for ongoing planning, organization, direction, evaluation and safe delivery of high-quality care to adult, geriatric, pediatric, and neonatal clients in accordance with Policies and Procedures, State and Federal Regulations. The role involves working 24 hours per week while being paid for 32 hours per week.

Requirements

  • A Baccalaureate Degree from an NLN accredited School of Nursing and a minimum of two (2) years of full-time clinical experience within the past five (5) years in a home care agency or related health care facility which included care of theונה
  • A Diploma in nursing or an associate’s degree in nursing and A. A minimum of three years of full time or full time equivalent clinical experience in nursing within the past five years, at least one of which was in a home health agency or community health program which included care of the sick at home; and B. Evidence of certification by the American Nurses’ Association as a community health nurse or completion of at least six credits received within two years in community health nursing theory or six credits in health care management from an accredited college or university program or school of nursing.
  • Knowledge of State Regulations, Medicare and Medicaid standards as related to Hospice services.
  • Current Connecticut Registered Nurse License.
  • Maintains current Drivers’ License and auto insurance.
  • Possess reliable transportation (Exception: Employee who utilize public transportation—Supervisory approval needed).

Responsibilities

  • Accepts appropriate referrals to the agency and assigns qualified professional personnel to meet client needs.
  • Plans and organizes daily assignments, including the review and assignment of new patient referrals.
  • Prepares on-call schedules in conjunction with clinical staff.
  • Supervises all assigned hospice and palliative nursing personnel in the delivery of nursing services.
  • Assigns work to subordinate staff, provides direction, and reviews work.
  • Participates in and provides coordination and management of all professional services rendered to clients and families by agency staff.
  • Supervises the maintenance of a balanced caseload and organizes the flow of work.
  • Utilizes clinical staff and makes field assessments as needed.
  • Performs periodic home visits with staff for role modeling, problem-solving, supervising, and evaluating performance/quality of care.
  • Assumes an active role in the professional development of assigned staff, including orientation, ongoing skill development, and mentoring.
  • Maintains an office environment that fosters professional growth and development of assigned staff.
  • Participates in staff recruitment, selection, and discipline/termination of employment in conjunction with Human Resources.
  • Conducts clinical record reviews to assess appropriateness of care plan, utilization of services, and completeness of documentation.
  • Participates in program planning and evaluation activities related to clinical services.
  • Demonstrates timely and accurate computer entry of patient/job information for OASIS data set, hospice item sets, orders for service, time sheets, and other required documents.
  • Participates in Administrative Call on a rotating basis.
  • Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities, and physical condition.
  • Reports to work on time, uses time off appropriately, and completes annual education & medical requirements.
  • Performs related and other duties as required.
