Requirements
- Minimum of one year nursing experience - medical/surgical, oncology, geriatric or related experience, hospice/home care experience preferred
- N. required; BSN Degree preferred
- CHPN or OCN preferred, BLS training
- NJ driver’s license and insurance coverage required
Nice To Haves
- Bilingual is a plus!
Responsibilities
- Perform patient assessments for comfort and relief symptoms
- Establish and implement an individualized plan of care for each patient
- Assess the need for patient and family education and provide education as needed.
- Obtain appropriate physician orders to achieve symptom management
- Triage and arrange home visit schedule, demonstrating efficiency in time and mile management
- Make referrals to additional IDT members and collaborate with social workers related to their involvement.
