About The Position
RN On-Call Baylor Nurse– Polk County Territory Compassionate Care Hospice (CCH), an Amedisys Company This position is based in Lakeland, FL and requires travel to assigned area(s) in Polk County. Schedule: Friday, Saturday and Sunday, 8am - 8pm. At CCH, we go above and beyond—not only for our patients but for each other. Requirements
- Current RN license in the state(s) where you will work
- Minimum 1 year of clinical RN experience in an acute healthcare setting (Required)
- Current CPR certification

- Hospice or home health experience
- Strong communication and interpersonal skills
- Ability to work independently and manage multiple priorities
- Comfortable with daily travel between facilities
- Proficient in electronic medical record (EMR) systems
Responsibilities
- Performs initial nursing assessment on new admissions.
- Completes, coordinates and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
- Regularly communicates patient progress to the clinical manager, physician and care team.
- Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
- Records pain/symptom management changes/outcomes as appropriate.
- Educates families on patient condition, needs and care.
- Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
- Performs on-call responsibilities and on-call services to patients/families as assigned.
- Performs other duties as assigned.
