About The Position

Masonicare Home Health & Hospice, a not-for-profit healthcare provider, is seeking a dedicated and accomplished RN Hospice Specialty Registered Nurse. The RN Hospice Nurse ensures the quality and safe delivery of hospice nursing services to patients in their homes or assisted living settings. This role involves developing, implementing, and evaluating the plan of care, making necessary revisions, and coordinating care with other health professionals. The nurse will identify interdisciplinary care needs and recommend additional home health services. Additionally, Hospice Nurses supervise care provided by Licensed Practical Nurses and Home Health Aides, conduct supervisory visits, and provide education, supervision, and counseling to clients and their families regarding home nursing procedures and other care needs. They ensure client and family involvement in the plan of care. Masonicare utilizes a fully electronic system, requiring timely and accurate computer documentation of patient information for OASIS data sets, orders, time sheets, and reports. Hospice Nurses are eligible for weekly productivity bonuses.

Requirements

  • Current RN License required.
  • NEW GRADS WILL NOT BE CONSIDERED.

Nice To Haves

  • Hospice AND/OR Home Care experience IS PREFERRED.

Responsibilities

  • Ensures quality and safe delivery of hospice nursing services to patients in the home or assisted living setting.
  • Develops, implements, and evaluates the plan of care and makes necessary revisions.
  • Provides coordination of the plan of care, maintaining continuity of client care with other health professionals.
  • Identifies interdisciplinary care needs and makes recommendations for additional home health care services.
  • Supervises the client care rendered by Licensed Practical Nurses and Home Health Aides in the provision of delegated duties and conducts supervisory visits per regulatory guidelines.
  • Provides education, supervision, and counseling to the client and their family regarding home nursing procedures and other care needs.
  • Assures that the client and their family are involved in the development of the plan of care whenever possible.
  • Demonstrates timely and accurate computer documentation of patient/job information.
