About The Position

As a Hospice Nurse, you will be responsible for overseeing the quality, expert care of your assigned hospice patients. Samaritan is the leading not-for-profit, locally based provider of home-based primary care, palliative care, hospice care, grief support, education, social connections and advocacy. Proudly serving the South Jersey community since 1980.

Requirements

• Minimum of two years of nursing experience.
• NJ RN license required

Nice To Haves

• BSN preferred
• Bilingual is a plus!

Responsibilities

• Perform patient assessments for comfort and relief of symptoms.
• Establish and implement an individualized plan of care for each patient.
• Assess the need for patient and family education and provide education as needed.
• Obtain appropriate physician orders to achieve symptom management.
• Triage and arrange home visit schedule, demonstrating efficiency in time and mile management.
• Make referrals to additional IDT members and collaborate with social workers related to their involvement.
