Cleveland Clinic’s Business Operations Center serves as a key hub for our Home Care and Hospice operations as well as many administrative and support teams. This collaborative workplace supports a wide range of caregivers, clinical professionals, and operational teams. While many positions are based within the center, numerous home-visiting roles—including case managers, nurses, and therapists—travel throughout the Greater Cleveland and Akron/Canton areas to deliver exceptional care directly to patients in their homes. Keeping patients connected to the highest quality care as they transition from the hospital to home or a facility is part of the Cleveland Clinic innovative approach to value-based care. As a Hospice Coordinator, you will be at their side the whole way, ensuring a smooth adjustment to their new routine. Through our Center for Connected Care, you will help provide a full continuum of care by processing inpatient referrals, coordinating discharge plans, coordinating services and more. Your efforts directly enhance the quality of life for patients and their families every day. A caregiver in this position works 8:00am to 4:30pm with every 4th weekend and 2 holidays per year (1 summer and 1 winter). This position requires travel to West Cleveland Clinic hospitals and facilities.
