About The Position

Amedisys Hospice Care is seeking an RN Field Case Manager – Hospice for South Miami-Dade County. This position is based in Hialeah, FL and requires daily travel throughout South Miami-Dade County. The schedule is Monday-Friday, 8:00 AM - 5:00 PM with participation in an on-call rotation. A key requirement is being fully bilingual in English and Spanish, with the ability to communicate effectively with patients, families, and team members in both languages. Requirements

Full bilingual proficiency in English and Spanish
Current RN license in the state(s) where you will work
Minimum 1 year of strong clinical RN experience in an acute healthcare setting
Current CPR certification

Nice To Haves

Hospice or home health experience
Strong communication and interpersonal skills
Ability to work independently and manage multiple priorities
Comfortable with daily travel between facilities
Proficient in electronic medical record (EMR) systems

Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
Records pain/symptom management changes/outcomes as appropriate.
Teaches patients, families, and caregivers appropriate patient care techniques.
Regularly communicates with clinical manager and care team.
Performs on-call responsibilities and on-call services to patients/families as assigned.
Other duties as assigned.
