Requirements

Active and valid RN license in the state of Ohio.
Minimum of two years of nursing experience required.
Demonstrated understanding of hospice philosophy and end-of-life care.
Strong communication skills and ability to engage effectively with patients, families, and colleagues.
Valid Ohio driver’s license and reliable transportation for patient visits.
Ability to work independently and manage multiple priorities.
Comfort working with patients experiencing terminal illnesses and providing emotional support to their families.
Ability to pass a pre-employment background check
Ability to pass a pre-employment drug screen

Nice To Haves

Prior experience in Med/Surg, Home Health, or Hospice settings is preferred.

Responsibilities

Provide compassionate care focused on comfort and quality of life for patients nearing the end of life.
Develop, implement, and manage individualized patient care plans under hospice protocols.
Conduct regular patient visits according to the established schedule, assessing patient needs and adjusting care plans.
Educate patients and families on end-of-life care, disease progression, and available resources.
Coordinate with interdisciplinary hospice team members, including social workers, chaplains, and therapists, to deliver comprehensive patient-centered care.
Administer medications and treatments as prescribed, monitoring for effectiveness and managing symptoms.
Document patient progress, changes in condition, and any interventions in the medical record promptly and accurately.
Communicate proactively with patients, families, and the care team to address concerns and provide emotional support.
Participate in hospice team meetings and contribute to developing patient care strategies.
Comply with all state, federal, and regulatory guidelines related to hospice care.
