Requirements

  • Active and valid RN license in the state of Ohio.
  • Minimum of two years of nursing experience required.
  • Demonstrated understanding of hospice philosophy and end-of-life care.
  • Strong communication skills and ability to engage effectively with patients, families, and colleagues.
  • Valid Ohio driver’s license and reliable transportation for patient visits.
  • Ability to work independently and manage multiple priorities.
  • Comfort working with patients experiencing terminal illnesses and providing emotional support to their families.
  • Ability to pass a pre-employment background check
  • Ability to pass a pre-employment drug screen

Nice To Haves

  • Prior experience in Med/Surg, Home Health, or Hospice settings is preferred.

Responsibilities

  • Provide compassionate care focused on comfort and quality of life for patients nearing the end of life.
  • Develop, implement, and manage individualized patient care plans under hospice protocols.
  • Conduct regular patient visits according to the established schedule, assessing patient needs and adjusting care plans.
  • Educate patients and families on end-of-life care, disease progression, and available resources.
  • Coordinate with interdisciplinary hospice team members, including social workers, chaplains, and therapists, to deliver comprehensive patient-centered care.
  • Administer medications and treatments as prescribed, monitoring for effectiveness and managing symptoms.
  • Document patient progress, changes in condition, and any interventions in the medical record promptly and accurately.
  • Communicate proactively with patients, families, and the care team to address concerns and provide emotional support.
  • Participate in hospice team meetings and contribute to developing patient care strategies.
  • Comply with all state, federal, and regulatory guidelines related to hospice care.
