About The Position

RN Field Case Manager – Hospice (North Miami-Dade County) Amedisys Hospice Care. This position is based in Hialeah, FL and requires daily travel throughout North Miami-Dade County. Schedule: Monday-Friday, 8:00 AM - 5:00 PM with participation in an on-call rotation. Requirement: Must be fully bilingual in English and Spanish, with the ability to communicate effectively with patients, families, and team members in both languages. You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience. Requirements

  • Full bilingual proficiency in English and Spanish
  • Current RN license in the state(s) where you will work
  • Minimum 1 year of strong clinical RN experience in an acute healthcare setting
  • Current CPR certification

Responsibilities

  • Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
  • Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
  • Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Teaches patients, families, and caregivers appropriate patient care techniques.
  • Regularly communicates with clinical manager and care team.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Other duties as assigned.
