RN Hospice Case Manager, Full Time, $5,000 retention bonus!
About The Position
Providence at Home, a growing division of Providence Life Services, is seeking a compassionate RN Hospice Case Manager to join their established and experienced hospice team in St. John, IN, and throughout Lake County. This full-time position focuses on providing exceptional, patient-centered care to individuals and families facing end-of-life. A $5,000 retention bonus is currently being offered, paid out in increments upon reaching milestone hours. The schedule is Monday - Friday, with one on-call night shift per week required. The role involves making a difference by providing dignity, comfort, and peace to patients in their final days.
Requirements
- Active Registered Nurse (RN) License in the state of Indiana.
- At least 1 year of RN experience, preferably in Hospice or Home Health.
- Driver's License in good standing.
- Reliable transportation.
Responsibilities
- Provide skilled nursing care.
- Provide case management services.
- Conduct initial assessments of new patients.
- Travel to patients wherever they reside: Home, nursing facility etc.
