Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, you’ll be the face of our care—helping families say “yes” to compassionate end-of-life support when it matters most. This role involves serving as a key ambassador for hospice services, visiting patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care. You will explain the hospice philosophy and services clearly, compassionately, and with clinical confidence, guiding patients and their loved ones through the admissions process to ensure a smooth and supportive start to care. Collaboration with referring physicians, case managers, and hospital or facility staff is essential for coordinating transitions of care. The role requires using the nursing process to assess patient needs, support care planning, and document clinical findings. Building trust and providing emotional reassurance to patients and families during this important decision-making process is crucial. Accurate completion of required documentation and ensuring compliance with state and federal regulations are also key responsibilities. Close work with the clinical and interdisciplinary team is necessary to ensure timely admissions and quality care handoff.
