Admission Nurse is responsible for identifying the needs of terminally ill patients and their families/caregivers throughout the referral, intake, and admission process. The nurse collects referral and insurance information as necessary and schedules admissions as appropriate. The RN meets with patients and caregivers for informational and admission visits as assigned and collaborates with physicians to establish the initial plan of care and meet immediate patient needs. The Admission Nurse works closely with hospital Case Management, inpatient care teams, and other referral sources to facilitate timely, coordinated transitions to hospice services, ensuring a seamless continuum of care and minimizing delays in admission.
