RN Homecare Field Case Manager
About The Position
Patriot At Home is a veteran-owned and family-operated company seeking a dedicated RN Homecare Field Case Manager for a full-time position in the Cincinnati area (Cheviot, Westwood, and surrounding areas). This role offers a flexible schedule with business hours of Monday - Friday, 8:00 AM to 4:30 PM. Additionally, the company utilizes Roger AI, an application designed to significantly reduce documentation time, allowing nurses to focus more on patient care and personal time.
Requirements
- Graduate from an accredited School of Nursing
- Licensed in the state as a Registered Nurse
- Knowledge of Medicare and Medicaid guidelines
- Criminal background check
- Must have a current CPR (BLS) certification
- Valid driver's license required
- Ability to access patients' homes, which may not be wheelchair accessible.
- Sufficient hearing, eyesight, and physical dexterity for patient assessments and care
- Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
Nice To Haves
- One or more years of experience in community/home health agency or hospital preferred
Responsibilities
- Have coordination of total patient care
- Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis
- Perform admission, transfer, recertification, resumption of care, and discharge Oasis for the home care patient
- All other duties as assigned
