About The Position

The RN Case Manager is responsible for performing visits and coordinating client care. They serve as the primary point of contact for all disciplines involved in patient care and manage the frequency of visits for each patient episode. This role involves consulting with physicians and the office regarding patient care, performing accurate OASIS collection, maintaining current medication profiles, ensuring timely reporting of lab values to physicians, and participating in weekly case conferences and monthly case manager meetings. The RN Case Manager must provide patient care that respects patient rights, dignity, and needs, assess physical, psychological, and social needs, manage pain effectively, and document all aspects of patient care according to guidelines, including reviewing patient history, diagnostics, and lab data for abnormalities.

Requirements

  • Current, unencumbered RN or LPN license
  • Certification in CPR
  • Prior clinical experience
  • Excellent communication skills
  • Demonstrated ability to maintain a high level of professionalism during stressful times
  • Strong critical thinking skills
  • Ability to adapt to different clinical settings and environments

Responsibilities

  • Perform visits and complete coordination of client care.

  • Provide all patient care in a manner that reflects respect for patient rights, dignity, and expressed needs.
  • Assess the patient’s physical, psychological, and social needs.
  • Assess pain and implement appropriate pain management techniques.
  • Document patient history, symptoms, assessments, tests, and care according to guidelines.
  • Review patient history, diagnostics, and lab data, and report any abnormalities.
