About The Position

The RN Case Manager is responsible for performing visits and coordinating client care. This role serves as the primary point of contact for all disciplines involved in patient care and oversees the frequency of visits for each patient's episode of care. The RN Case Manager also consults with physicians and the office regarding patient care details, performs accurate OASIS collection, maintains current medication profiles, ensures timely reporting of lab values to physicians, and participates in weekly case conferences and monthly case manager meetings. All patient care must be delivered in a manner that respects patient rights, dignity, and expressed needs. This includes assessing the patient’s physical, psychological, and social needs, assessing and managing pain, and documenting patient history, symptoms, assessments, tests, and care according to guidelines. Reviewing patient history, diagnostics, and lab data, and reporting any abnormalities are also crucial aspects of this role.

Requirements

  • Current, unencumbered RN or LPN license
  • Certification in CPR
  • Prior clinical experience
  • Excellent communication skills
  • Demonstrated ability to maintain a high level of professionalism during stressful times
  • Strong critical thinking skills
  • Ability to adapt to different clinical settings and environments

Responsibilities
