About The Position

The Home Infusion Clinical Services Liaison – RN, is responsible for positive and effective representation of the Mount Sinai Home Infusion Program and introducing the full scope of services offered to hospital case managers, physicians, provider staff, nursing agencies, other referral sources, and patients and their caregivers. This role involves providing education about the program and services to referral sources and patients, and assisting with the clinical coordination of care between referral sources, the pharmacy, nursing agency, and patient/patient caregivers, on behalf of Mount Sinai Home Infusion. The liaison will utilize clinical knowledge and skills to assist referral sources and/or patients with identifying appropriate candidates for home infusion, confirming patients’ and/or caregivers’ ability to receive care in the home, patient education and training regarding therapy and supplies, clarifying expectations of care in the home, and medication connection at the hospital or facility to expedite discharge into the home setting, as appropriate.

Requirements

  • Registered Nurse (RN)
  • Clinical knowledge and skills related to home infusion therapy
  • Ability to educate patients and caregivers
  • Ability to coordinate care between multiple parties
  • Familiarity with identifying appropriate candidates for home infusion
  • Ability to assess patient/caregiver readiness for home care
  • Experience with medication connection for discharge planning

Responsibilities

  • Providing education about the program and services to referral sources and patients.
  • Assisting with clinical coordination of care between referral sources, the pharmacy, nursing agency and patient / patient caregivers, on behalf of Mount Sinai Home Infusion.
  • Providing and utilizing clinical knowledge and skills to assist referral sources &/or patients with: The identification of appropriate candidates for home infusion, Confirming the patients’ and/or caregivers’ ability to receive care in the home, Patient education and training regarding therapy and supplies, as well as, clarifying expectations of care in the home, Medication connection at the hospital (i.e. inotropes) / facility in order to expedite the discharge into the home setting, as appropriate.
