Requirements
- Current, unrestricted RN license
- 1+ year clinical nursing experience
- Willingness to travel within assigned territory (up to 50%)
- Valid driver’s license, reliable vehicle, and current auto insurance
- Ability to lift/push/pull up to 50 lbs and assist with patient mobility
Nice To Haves
- Associate’s or Bachelor’s in Nursing preferred
- Home Health, Hospice, or HomeCare HomeBase experience preferred
Responsibilities
- Align care with Elara Caring mission and leadership direction
- Maintain HIPAA confidentiality standards
- Assess patient and family needs (physical, psychosocial, environmental)
- Manage full caseload including planning, coordinating, and evaluating care plans
- Perform initial and ongoing patient evaluations and update care plans
- Communicate changes in patient condition to clinical leadership and care team
- Revise and coordinate care plans with interdisciplinary team
