About The Position

Location: Altru Business Center 2401 Demers Ave Grand Forks, ND 58201 Summary: The RN Home Health provides care which is appropriate to the patient's diagnosis, symptoms, and physicians orders. This position directs and oversees care provided by Licensed Practical Nurse (LPN) and aides. The RN establishes and implements documented plans of care, determines appropriate outcomes/goals and communicates/collaborates with patient, interdisciplinary team, and physicians in providing care.Supervised by the Supervisor and Manager of Home Health.

Requirements

  • Registered Nurse | North Dakota Board of Nursing (NDBON) or Compact State Agreement | Prior to Start Date | HR Primary Sources
  • Registered Nurse | Minnesota Board of Nursing | Within 12 Months of Start Date | HR Primary Sources
  • Basic Life Support (BLS) | American Heart Association (AHA) | Within 90 days of Start Date | Learning Management System
  • Driver's License with Acceptable Driving Record | Drivers License - Current State Licensed | Prior to Start Date | HR Primary Sources
  • Minnesota Background Study | Minnesota Department of Human Services | Start Date
  • Temporary RN license accepted prior to start.
  • Must hold an unencumbered nursing license prior to start.
  • If Minnesota license was issued prior to 2018, must complete MN Background study with HR Compliance
  • Ensures access to reliable personal transportation for work-related travel as needed.
  • Required: A minimum of 1 year Related Experience
  • This position requires proficiency in reading, writing, and speaking English to ensure effective communication in the workplace and with patients, families, and team members.

Responsibilities

  • Provides care which is appropriate to the patient’s diagnoses, symptoms, and physician orders.
  • Directs and oversees care provided by Licensed Practical Nurse (LPN) and aides.
  • Establishes and implements a documented plan of care.
  • Determines appropriate outcomes/goals.
  • Communicates and collaborates with the patient, interdisciplinary team, and physicians in providing care.
  • Promotes patient/caregiver education using various verbal and written communication techniques that take into account the patient’s/caregiver’s cultural, ethnic, and personal needs or preferences
  • Documents patient/caregiver responses to care, treatment, and services, including progress towards goals.
  • Identifies discharge planning needs as early as possible and initiates needed referrals to other disciplines, alternative level of care, or community resources as needed.
  • Complies with Joint Commission, federal and state regulations, reimbursement guidelines, and Altru and Altru Home Services policies and procedures.
  • Performs pain assessment and takes appropriate action according to standard operating procedure.
  • Performs other duties as assigned or needed to meet the needs of the department/organization.
