About The Position

Enhabit Home Health & Hospice is hiring a full-time weekend Registered Nurse for home health care. This field-based role performs skilled nursing visits and coordinates patient care. The schedule is flexible, but Saturday and Sunday work is required.

Requirements

• Hold a current Registered Nurse (RN) license in the state of employment.
• Demonstrate knowledge and skill in current nursing practice.
• Have a valid state driver’s license and automobile liability insurance.

• A minimum of one year of clinical experience is preferred.

Responsibilities

• Perform skilled nursing visits and coordinate client care.
• Serve as the point of contact for all disciplines involved in providing patient care.
• Oversee the frequency of visits during the episode of care.
• Consult with the physician and office as needed, providing details about patient care.
• Complete accurate OASIS collection and keep the medication profile current.
• Ensure lab values are reported to the physician promptly.
• Attend weekly case conferences and monthly case manager meetings.
