About The Position

A healthcare organization is seeking an experienced Home Health Registered Nurse for a 13-week assignment. The RN will provide comprehensive in-home patient care, ensuring high-quality clinical outcomes through assessment, planning, implementation, and evaluation of individualized care plans. The coverage area includes Brewster and Harwich. Candidates must secure housing within the Cape Cod region (mid-Cape or closer). This is a field-based home health role with a focus on quality outcomes and patient-centered care within a collaborative and supportive clinical environment.

Requirements

• Active Massachusetts RN License
• Minimum 1 year of nursing experience (or 6 months with BSN)
• CPR Certification
• Successful completion of pre-employment medication test

Nice To Haves

• Home Health or IV therapy experience
• BSN degree
• Strong computer and documentation skills

Responsibilities

• Perform comprehensive patient assessments, including physical, psychosocial, environmental, and health history evaluations
• Develop and implement individualized care plans based on assessment findings
• Monitor and evaluate patient progress, revising care plans as needed
• Maintain documentation that accurately reflects patient care, teaching, and outcomes
• Coordinate care with interdisciplinary teams and participate in case conferences
• Supervise Home Health Aides and review care plans every 14 days
• Communicate significant patient condition changes to physicians and care team members
• Obtain and verify physician orders for all care provided
• Plan and coordinate patient discharges and complete required documentation
• Refer patients to community resources when appropriate
• Ensure compliance with clinical standards, benchmarks, and regulatory requirements
