About The Position

We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in a home care setting. The ideal candidate will perform comprehensive patient assessments, develop individualized care plans, coordinate interdisciplinary services, and ensure high-quality patient-centered care while supporting patient independence and safety.

Requirements

  • Current Massachusetts RN License required
  • Current CPR Certification required
  • Successful completion of pre-hire medication test required
  • Minimum 1 year of nursing experience required OR Minimum 6 months of nursing experience with a BSN
  • IV therapy experience preferred
  • Computer skills preferred
  • Strong clinical assessment and critical-thinking abilities
  • Excellent communication and patient education skills
  • Ability to work independently and manage patient caseloads effectively
  • Strong organizational and documentation skills
  • Compassionate, patient-focused approach to care
  • Ability to collaborate effectively with interdisciplinary healthcare teams
  • Commitment to delivering high-quality home health services

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred
  • Home Health or Home Care experience preferred

Responsibilities

  • Perform comprehensive nursing assessments upon patient admission, including physiological, psychosocial, and environmental evaluations
  • Develop and implement individualized patient care plans based on assessment findings and case management objectives
  • Monitor patient progress and evaluate the effectiveness of care plans and interventions
  • Participate in interdisciplinary patient case conferences and coordinate care with healthcare teams
  • Provide documented supervision of Home Health Aide care plans every 14 days
  • Notify physicians of significant patient condition changes and obtain/clarify medical orders as needed
  • Plan and coordinate patient discharge with complete and accurate documentation
  • Deliver skilled nursing care, patient education, and caregiver instruction
  • Maintain clear, concise, and timely clinical documentation reflecting patient care and responses to treatment
  • Ensure compliance with home health regulations, safety standards, and patient confidentiality policies
