About The Position
The Registered Nurse (RN) in Home Health provides comprehensive care to adult patients, managing approximately 30 cases using the Homecare Homebase (HCHB) system. The role includes "start of care," recertifications, and daily visits with a focus on Home Health services.
Requirements
- Licensed in the State of work.
- Must have 1 year of experience on a Medsurg floor in a hospital setting.
- Experience with Homecare Homebase (HCHB) system required.
- BLS (AHA) certification required.
Responsibilities
- Manage a caseload of approximately 30 patients, achieving 6.5 points per day.
- Perform "start of care," recertifications, and daily visits within Home Health.
