About The Position
ATC Healthcare is seeking an experienced Home Health & Hospice RN for a 13-week travel assignment. This role involves providing skilled nursing care to patients in homes, nursing facilities, and community settings within a 60-mile radius of Dickinson. Candidates must have prior Home Health and Hospice experience. Mileage is not paid separately and is factored into the bill rate. On-call rotation for evenings, weekends, and holidays is required.
Requirements
- Active RN license required
- Home Health & Hospice experience required
- BLS certification required
- Current Driver’s License required
- Proof of Auto Insurance required
- Must meet 50-mile radius rule
- Cannot be employed with CommonSpirit/CHI/Dignity facilities within the past year
Responsibilities
- Perform initial assessments and develop individualized plans of care
- Conduct ongoing reassessments and revise care plans as needed
- Administer medications and treatments as prescribed
- Coordinate care as Case Manager when assigned
- Provide patient and family education
- Communicate with physicians regarding patient condition and orders
- Arrange necessary equipment and services
- Supervise and evaluate home health aide services
- Complete documentation in HomeCare HomeBase EMR
