About the Home Health/Hospice Team Our Home Health and Hospice teams pride themselves on providing top quality care in the home to those individuals requiring additional healthcare services or during their end-of-life journey. Mission for Position The Home Health/Hospice Admission RN enhances quality of life for home care and hospice patients by delivering skilled nursing care, promoting comfort and dignity, and partnering with patients, families, and the care team to support holistic care.

Requirements

• Current Registered Nurse (RN) license in the State of Minnesota.
• Valid Minnesota driver’s license and current auto insurance for travel between patient homes.

Nice To Haves

• Experience in home health or hospice nursing preferred.
• Strong patient assessment, care planning, and clinical decision-making skills.
• Ability to educate patients and families on disease processes, symptom management, and end-of-life care.
• Comfortable working independently in home environments while collaborating with an interdisciplinary care team.
• Strong organizational, communication, and documentation skills, including proficiency with electronic medical records.
• Knowledge of home care and hospice regulations and best practices.

Responsibilities

• Conduct comprehensive nursing assessments during the Home Care/Hospice Admission Visit
• Review and correctly implement the patient ’ s plan of care, as established by the Physician
• Identify and initiate appropriate interventions for immediate care needs during the admission process
• Confirm eligibility for Home Care and Hospice services based upon regulatory requirements
• Observe proper infection control and safety procedures
• Supervise health assistant services
• Coordinate care team to meet patient needs
