About The Position
The RN Home Health Clinical Supervisor is responsible for completing initial comprehensive and ongoing assessments of patients and families to determine the best services to meet their needs. This role involves coordinating services with the patient, family, and care team, providing ongoing supervision of all professional and para-professional staff assigned to the patient, and maintaining ongoing communication with the patient and family to ensure client satisfaction and optimal outcomes.
Requirements
- Registered Nurse (RN) license NYS
- BSN required
- 3 + years of clinical experience
- Home care experience
- Minimum 1 year of supervisory experience
- Excellent observation, problem solving and communication skills
Responsibilities
- Completes initial comprehensive and ongoing assessment of patient and family to determine best services to meet their needs
- Coordination of services with the patient, family, and care giver team.
- Ongoing supervision of all professional and para-professional staff assigned to the patient.
- Ongoing communication with patient and family to ensure client satisfaction and best outcomes
