About The Position

This position is a dynamic and hands-on, rewarding position effecting positive and life-giving care to 3–4 high-acuity individuals. The ideal candidate will love people and thrive on being part of meaningful moments that help each client maximize their quality of life despite injury, illness, or disability. In addition to hands-on care, this role will train on the basics of case management in a supportive environment, with one designated day per week devoted to instruction, collaboration and practice. You will receive training in assessments, care plans, EMR documentation, collaborating with other care providers for the client, supply management, communications and team building. This position is the ideal environment for training for higher level case management and leadership positions within the company. Requirements

Position is IN-PERSON ONLY (NOT REMOTE) providing meaningful, hands-on care
Six (6) months experience as an RN providing direct patient care
Must be eligible to work in the United States
Current CPR Certification
Documentation of negative TB test (Mantoux/ blood test/chest x-ray) prior to or upon hire
Current MN Driver's License and reliable transportation
Excellent organization, planning and communication skills
Proficiency with Electronic Medical Records (EMR) software or the ability and enthusiasm to quickly learn
A positive, team-oriented mindset and a genuine passion for improving the lives of individuals with complex medical needs
