About The Position
Suncoast Hospice, a part of Empath Health is seeking a caring and motivated RN Case Manager to work with us in Pinellas County while providing exceptional care on the following schedule: Monday-Friday 8a-5p with occasional weekend day, 1 summer and 1 winter holiday requirement. Coverage territory Pinellas primarily with Hillsborough possibility. Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way.
Requirements
- Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
- Minimum one year experience in an acute-care hospital environment providing direct patient care.
- Valid Florida State Driver’s License
Nice To Haves
- previous experience as a hospice RN preferred.
Responsibilities
- Reviews and updates the care plan, physician orders, pharmacy profile and other records.
- Educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.
