About The Position
E & S Home Care Solutions, LLC is seeking a compassionate, reliable, and experienced Registered Nurse (RN) Field Nurse to join its home care team. This field-based role involves working independently and conducting in-home assessments to help ensure clients receive quality care in their homes. The position offers flexible scheduling and additional income while making a meaningful difference in clients’ lives. Compensation is $35–$45 per completed assessment, based on experience and assessment type. Mileage reimbursement is $0.76 per mile for travel exceeding 20 miles one way.
Requirements
- Active and unrestricted Registered Nurse (RN) license in New Jersey.
- Strong nursing assessment and documentation skills.
- Excellent communication and organizational skills.
- Ability to work independently and manage field visits efficiently.
- Valid driver’s license and reliable transportation.
- Compassionate, dependable, and professional.
Nice To Haves
- Previous home care or home health experience.
Responsibilities
- Conduct initial nursing assessments and reassessments in clients’ homes.
- Develop, review, and update individualized plans of care.
- Perform supervisory visits for Home Health Aides (HHAs).
- Evaluate clients’ health conditions and identify changes in care needs.
- Ensure compliance with New Jersey home care regulations and agency policies.
- Complete required nursing documentation accurately and within established deadlines.
- Communicate with clients, families, caregivers, and office staff.
- Provide HHAs with guidance and education regarding client care needs.
