The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. This role involves completing clinical admissions, explaining services, identifying admission objections, educating patients/families, completing consents and assessments, providing information to physicians, initiating skilled nursing interventions, obtaining physician orders, and completing documentation. The RN also acts as a company representative at assigned facilities, facilitates referrals, communicates with admissions, obtains medical record information, develops relationships with case managers and physicians, and provides education on available services. They also provide care for GIP patients in hospitals, including daily visits, professional management, documentation, and efficient discharge. The role requires adherence to company policies, regulations, and quality improvement activities.
