About The Position

Lightways Hospice and Serious Illness Care, an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois, offers exceptional serious illness care, hospice care, and grief support for adults and children. The majority of our direct patient care is delivered in the field, with team members providing care in homes, nursing facilities, or assisted living facilities. We also operate a 24-bed inpatient unit in Joliet, and our non-clinical staff are based at our Joliet administrative offices.

Requirements

Current RN license
Graduate of accredited nursing program
Minimum of 2 years of nursing experience
Valid driver’s license and insured, reliable vehicle

Nice To Haves

At least 1 year in public health, home care, or hospice preferred

Responsibilities

Deliver skilled nursing care and hospice services in home and community settings
Conduct comprehensive patient assessments and develop individualized care plans
Administer medications and treatments while monitoring patient symptoms and outcomes
Educate patients and caregivers on care, self-management, and disease progression
Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
Maintain accurate documentation and participate in on-call coverage as needed
Cover DuPage and Southern Cook County (Tinley Park, Orland Park, Downers Grove, Lemont)
Monday–Friday schedule
Minimum of 20 on-call hours per month (may increase), supporting entire service area
