About The Position

Lightways Hospice and Serious Illness Care, an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois, offers exceptional serious illness care, hospice care, and grief support for adults and children. The majority of direct patient care is provided in the field, with team members caring for patients in their homes, nursing facilities, or assisted living facilities. They also operate a 24-bed inpatient unit in Joliet. Non-clinical staff are based at the Joliet administrative offices.

Requirements

Current RN license
Graduate of accredited nursing program
Minimum of 2 years of nursing experience
At least 1 year in public health, home care, or hospice preferred
Valid driver’s license and insured, reliable vehicle

Responsibilities

Deliver skilled nursing care and hospice services in home and community settings
Conduct comprehensive patient assessments and develop individualized care plans
Administer medications and treatments while monitoring patient symptoms and outcomes
Educate patients and caregivers on care, self-management, and disease progression
Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
Maintain accurate documentation and participate in on-call coverage as needed
Coverage may include portion of Will and Southern Cook Counties (Orland Park, Homer Glen, Lockport & Surrounding Areas)
Monday–Friday schedule
Minimum of 20 on-call hours per month (may increase), supporting entire service area
