About The Position
AccordCare Home Health is seeking a RN Field Case Manager - Nonskilled Division to join their Maitland, FL office. This role primarily involves overseeing non-skilled home care services, acting as a liaison between clients, caregivers, and referral sources, and ensuring high-quality client care. The position requires occasional in-office time and offers a healthy work-life balance within a positive, team-oriented environment.
Requirements
- Current RN license in the State of Florida (required)
- Reliable transportation and valid driver’s license for occasional field visits
- This role requires a screening through the Care Provider Background Screening Clearinghouse.
Nice To Haves
- 1+ year of experience in home care or community health preferred
- Excellent communication, leadership, and organizational skills
- Positive, professional attitude and a strong commitment to quality care
- Familiarity with AlayaCare software a plus
Responsibilities
- Manage and coordinate non-skilled home care cases, ensuring clients receive exceptional service and support.
- Serve as the main liaison between clients, caregivers, and referral sources.
- Oversee daily office operations, assisting with scheduling, authorizations, and client communications.
- Conduct aide orientations, trainings, and supervisory visits to promote quality and consistency in care.
- Monitor compliance with all AccordCare policies, as well as state and federal regulations.
- Collaborate with leadership to continuously improve client satisfaction and caregiver performance.
