About The Position

Responsible for managing care for complex gastroenterology patients to promote effective education, self-management support, care coordination, and timely healthcare delivery. Collaborates with patients, providers, and interdisciplinary teams to develop individualized care plans and improve patient outcomes.

Requirements

• Associate Degree in Nursing (ASN) or equivalent from an accredited institution required.
• Current New Hampshire Registered Nurse license and BLS/CPR certification required.
• Minimum of two years of professional nursing experience required.
• Strong clinical assessment, communication, and care coordination skills.
• Knowledge of electronic medical records, chronic disease management, and patient-centered care principles.
• Ability to manage multiple priorities, work independently, and collaborate effectively with patients, families, and care teams.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Ambulatory care, case management, population health, or care coordination experience preferred.

Responsibilities

• Provide care coordination and case management for patients with complex healthcare needs.
• Collaborate with providers and interdisciplinary care teams to develop individualized care plans.
• Educate patients on disease management, medications, nutrition, and self-management strategies.
• Coordinate transitions of care, including hospital discharges, specialty referrals, and follow-up care.
• Utilize population health tools and electronic medical records to identify and manage patients requiring additional support.
• Promote patient safety, quality outcomes, continuity of care, and preventive health initiatives.
