About The Position

This position is responsible for planning, directing, coordinating and providing high quality, individualized patient care using the components of the nursing process. The role involves assessing critically ill patients, developing and implementing care plans, administering medications, ensuring patient safety, and providing care in emergencies. It also includes managing patient rights, educating patients and families, and demonstrating effective team leadership. The position also involves understanding and contributing to Value Based Purchasing goals, including Core Measures, HCAHPS scores, and hourly rounding.

Requirements

  • Graduate of an accredited RN Training Program.
  • Minimum one-year recent experience in an acute care setting preferred.
  • Ability to communicate effectively verbally and in writing.
  • Must be able to work in a union environment.
  • Current California RN license
  • American Heart Association BLS card
  • American Heart Association ACLS card
  • Basic EKG
  • NIHSS (Stroke)
  • Assault Response Competency (ARC) required (within 30 days of hire)
  • Los Angeles County Fire Card (within 30 days of hire)

Responsibilities

  • Performs initial assessment in a timely manner and ongoing reassessments when warranted by the patient's condition, incorporating cultural and ethnic factors, differentiating between normal and abnormal findings, and incorporating functional/developmental age into assessment/reassessment for adolescent through geriatric patient population.
  • Demonstrates sound clinical judgment in the development, implementation, and evaluation of the nursing aspects of the trans-disciplinary care plan, developing an individualized patient care plan and determining interventions based on assessment/reassessment findings.
  • Evaluates patient outcomes on an ongoing basis, establishes priorities for patient care services as evidenced by modification of the care plan, if necessary, and accurately documents patient care activities in the medical record.
  • Utilizes a case management approach to plan, guide, direct, and coordinate patient care across the continuum by effectively communicating with physicians, staff, and other disciplines.
  • Safely and competently administers pharmacotherapeutics adhering to medication policies, procedures, practices and standards, and monitors the resultant outcomes based upon the patient's response to treatment.
  • Assesses and responds appropriately to ensure patient safety based on the physical environment, equipment, and the patient's behavior.
  • Demonstrates the ability to provide care in emergencies and follows established emergency procedures and practices.
  • Communicates and follows the organizational chain of command for notification of patient care/service issues, when appropriate.
  • Verbalizes and demonstrates the ability to obtain and interpret cardiac rhythms and the treatment of arrhythmia.
  • Maintains knowledge of, and demonstrates the ability to identify therapeutic patient care issues/problems and determines measures and interventions to promote, maintain and/or restore health for the adolescent through geriatric patient population.
  • Assures that the rights of all patients are respected and maintained by allowing for privacy and dignity in the provision of patient care.
  • Determines the presence and type of Advance Directive, if applicable, and ensures patient and/or family involvement in the decision-making process as it relates to patient care, end of life and ethical issues.
  • Reports suspected cases of abuse or neglect relative to the adolescent through geriatric patient population as appropriate.
  • Identifies patient/family educational needs; initiates and provides individualized teaching which considers the functional/developmental age and the needs of the adult patient population.
  • Evaluates and documents the effectiveness of the patient/family teaching through assessment of their comprehension of the instruction and demonstration of appropriate responses.
  • Demonstrates knowledge of age specific care, including identifying the need for additional safety measures, physiological normal values/readings; and assessing skin integrity, behavior, motor skills and/or activities that place patients at risk as well as communicates effectively in a clear, concise, understandable manner.
  • Demonstrates effective Team Leader/Charge Nurse skills.
  • Practices Universal Precautions.
  • Participates in the Medical Center's continuous quality improvement process.
  • Complies with the Medical Center's continuous quality improvement process.
  • Understands and verbalizes VBP goals/targets.
  • Consistently ensures Core measures for Department are implemented and documented.
  • Consistently participates in the unit and hospital specific action plans to improve HCAHPS scores.
  • Is able to use AIDET in all sittings.
  • Ensures completion of hourly rounding and updates white boards in a timely manner.
  • Understands is individually accountable for achieving Core Measures, HCAHPS and other VBP goals.
  • Reports to work on time and as scheduled, completes work within designated time.
  • Uses computerized time clock correctly.
  • Attends staff meetings and actively participates, reads and returns all staff meeting minutes.
  • Actively participates in performance improvement and continuous quality improvement (CQI) activities.
  • Demonstrates an understanding of the procedures for reporting and responding to facility emergencies such as fire, and disruption in utilities and hazardous materials spills.
  • Demonstrates knowledge of equipment capabilities, limitations, and appropriate/special applications.
  • Demonstrates the ability to access and use computer systems.
  • Communicates with physicians, administrative staff, co-workers, and patients/families in a respectful and caring manner.
  • Demonstrates a positive professional image; performs responsibilities in a professional manner; demonstrates flexibility in the provision of patient care/service; utilizes a collaborative approach with co-workers.
  • Demonstrates flexibility in the work setting with changing assignments, varying staffing levels and patient care environments, and completes work on time.
