About The Position

The RN, IDD Nurse Case Manager is responsible for direct, indirect patient care and services coordination. S/he participates in the case management and care coordination of individuals with developmental disabilities; monitoring referrals, managing prescription refill requests. S/he is an integral part of the IDD care team focusing on positive outcomes for patients.

Requirements

  • Current California RN license.
  • Current CPR certification.
  • Current ACLS certification
  • Minimum of three years of experience in direct patient care as an RN.
  • Knowledge of the nursing process and critical thinking abilities.
  • Knowledge of principles of nurse case management.
  • Knowledge of electronic medical records and tracking systems.
  • Demonstrates knowledge of compliance issues within the community clinic environment.
  • Demonstrates a willingness to report any incident that is unusual or incompatible with accepted clinic procedures.
  • Maintains confidentiality of patients at all times by complying with HIPAA policies.
  • Strong interpersonal skills; ability to be sensitive with persons of various social, cultural, economic, and educational backgrounds.
  • Experience with Microsoft Office applications including Outlook, Word, Excel, and PowerPoint.
  • Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
  • Strong written and verbal communication skills.
  • Ability to work independently and as part of a team.
  • Good judgment, problem solving and decision-making skills.

Nice To Haves

  • Minimum of one year of experience in developmental disability nursing preferred.
  • Fluency in English and Spanish (speak, read, and write) preferred.

Responsibilities

  • Assists with meeting the needs of individuals with disabilities, including assessment, education, referrals, and follow-up care. This may involve face-to-face, phone meetings and review of electronic messaging and media in clinical and home care settings.
  • Provides nursing care including but not limited to administering medications and treatments in compliance with a medical provider’s and standing orders, Santa Rosa Community Health policies, and State of California guidelines. This is inclusive of assigned functions within electronic health records.
  • Solo and shared nursing office visits for patients with IDD for specialized care/management (e.g. bladder scan, blood draws with desensitization methods, wound care, medication reconciliation)
  • Point person for triage for ARFPSHN and other specialized homes.
  • Medication reconciliation and chart review for ARFPSHN (weekly) and other specialized homes.
  • Interprets information obtained from patient’s health records, patients, and care givers, including the health care team, and documents/transcribes all clinical care in the patient’s electronic medical record.
  • Tracks referrals to IDD team and schedules for both establishing primary care and for consults within SRCH for IDD co-management.
  • Assist Clinicians and Care team in accessing care and coordinating referrals to outside community services and resources (includes connecting with NBRC service coordinators)
  • Provides general care coordination of case managed patients, including but not limited to patient triage, phone call-backs, prescription refills, lab ordering/ interpretation, and medical case management.
  • Scheduling and tracking IDD dental visits in coordination with dental scheduler and program manager – non-sedated visits, bravery coaching, OCS and VDH
  • Provides general care coordination and integration for Oral Conscious Sedation Program with Program Manager, including but not limited to: consent tracking, care plan tracking, scheduling, pre-visit planning monitoring patients before, during and after dental procedures, administering sedation and monitoring response, emergency management post-visit review with dental team.
  • Collaborates with Dental staff to ensure positive outcomes for IDD patients dental visits.
  • Performs education and counseling about disease management process and medication adherence.
  • Regular communication, patient advocacy and case conferencing with patient’s primary team of providers.
  • Participates in ongoing training and quality improvement projects.
  • Acts as a resource/contact person for clinical staff, service partners, and patients/caregivers for all of SRCH.
  • Maintains skills proficiency in general nursing.
  • Highly encouraged to pursue regular CMEs in neurodevelopmental nursing as well.
  • Performs related duties as requested.
