This role involves coordination with the care team and combined with virtual contacts to assess the needs of patients and their families. Responsibilities include developing, implementing, evaluating, and adjusting an appropriate Plan of Care based on the patient’s clinical needs. The Desk Case Manager identifies appropriate interdisciplinary services needed, and coordinates required services.
