About The Position

We are seeking a skilled RN with acute care, hospice, and oncology experience to join our team at the newly expanded Community Hospice House in Merrimack, NH. Our mission is to help patients and their families experience a dignified, peaceful end of life. Home Health & Hospice Care is one of the oldest VNAs in the US, founded in Nashua, NH in 1883. Requirements

• Graduate of an accredited school of nursing.
• Associate's degree in nursing.
• Strong skills in symptom management.
• One year recent acute care experience.
• Experience managing oncology patients and those with co-morbidities.
• Registered as a RN with New Hampshire’s Board of Nursing.
• CPR Certification.
• Satisfactory criminal record.
• Position requires sharing in every other weekend rotation and agency holidays.

Nice To Haves

• Bachelor’s degree in nursing.
• Community Health/Education experience.
• Certification in hospice and palliative care a plus.
• Knowledge or experience with end-of-life care and hospice care is beneficial.
• EMR software proficiency. We use EPIC.

Responsibilities

• Accessing, collecting data, and evaluating the patient's physical, spiritual, and emotional needs.
• Provides preventative and therapeutic treatment under medical direction.
• Creates a nursing plan of care with input from the patient, family, and provider to include measurable goals and behavioral objectives.
