This role involves visits to homes, to participants of the LIFE program. The organization offers mileage reimbursement for distances driven. Triages and assesses Participants in the home clinic setting and providing education resources. Participate as a member of the interdisciplinary team (IDT), including completion of initial medical history, physical exam, and functional nursing assessments of each new participant and semi-annual, annual, and unscheduled assessments; communicates changes in participant health or functional status to the interdisciplinary team members, and participates in development of the plan of care and coordination for 24-hour care delivery. Participates in daily program activities and provides comprehensive plan of care to meet participant needs and promote optimum health, independence and self-care. Communicates, counsels and guides participants and families regarding service needs and concerns; makes referrals as appropriate. Collaborates with Clinic RNs and RN Case Managers, and PCPs regarding care delivered to Participants in the clinic and any transitions of care, through verbal and electronic communication. Maintains current written/electronic records and documents related to participant care. Actively serves on the Quality Committee. Involved in the development and implementation of QAPI activities. Conducts comprehensive nursing assessment of participants, at appropriate intervals, at times of status changes Registered Nurse (RN) and as needed with each participant, family members and caregivers
