About The Position

The RN Clinical Supervisor is responsible for reviewing clinical documentation for compliance and completeness, ensuring accuracy of OASIS and Plan of Care (485), and completing medication reconciliation. This role involves reviewing and approving referrals, ensuring thorough Intake Communication notes, and communicating with Clinical Managers to ensure appropriate patient scheduling based on acuity. The supervisor will also communicate patient hospitalizations and other pertinent information to appropriate staff, follow patients until admission after a referral is received, and direct the patient care team to ensure high-quality care. Responsibilities include evaluating patient conditions for the need of additional services, equipment, and consultations, holding regular meetings for team education and interdisciplinary communication, and collaborating with clinicians to achieve positive medical outcomes while managing resources. The role also involves planning for discharge at the time of admission, potentially approving LPN verbal orders, understanding payer processes, and acting as a liaison between patients/families, physicians, and other healthcare providers. The supervisor will accept and enter workflow from authorized practitioners for tasks outside an LPN's scope of practice, provide in-home nursing visits as needed, participate in administrative on-call rotation, enter OASIS assessments, create/approve Home Health Aide care plans, understand patient outcome and HHCAHPS requirements, and review Telehealth data, collaborating with staff and physicians on identified negative results or trends. Other duties as assigned.

Requirements

  • Registered Nurse with a current, valid Pennsylvania license.
  • Two [2] years of progressive experience in a home health care setting preferred.
  • Interpersonal, communication and organizational skills.
  • Proficient with knowledge of an electronic health record.
  • Knowledge of agency policies, both federal and state regulations and third-party payers.
  • Reliable, independent means of transportation to make home visits.
  • Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.

Responsibilities

  • Reviews clinical documentation for compliance and completeness in a timely manner.
  • Reviews OASIS and Plan of Care (485) for accuracy and completeness.
  • Completes medication reconciliation, as appropriate.
  • Reviews and approves the referral for accuracy and completeness in the electronic medical record.
  • Ensures Intake Communication note is thorough and complete.
  • Communicates with the Clinical Managers and/or scheduling to ensure the patient is scheduled appropriately for their level of acuity.
  • Communicates all patient hospitalizations and other pertinent information to the appropriate staff.
  • Once a referral is received, follows patients until admission to the agency.
  • Directs members of the patient care team to assure that a high quality of care is provided.
  • Evaluation of patient’s condition for the need of other services, equipment, and consultations that may facilitate better overall outcomes.
  • Holds regular meetings to provide education to team members and facilitate interdisciplinary communication.
  • In collaboration with the primary clinician, provides what is necessary for the patient’s need to obtain positive medical outcomes while managing appropriate resources.
  • Plans with the primary clinician and other disciplines for the discharge from service at the time of admission.
  • May review and approve LPN verbal orders.
  • Knowledge about payer processes and requirements to bridge the gap between provider and payer and to effectively meet the needs of both, ultimately for the patient’s benefit.
  • Liaison between patient/family, physician and all other healthcare providers.
  • Accepts and enters into the medical record workflow from authorized practitioners (e.g. IV orders for IV push medication, chemotherapy agents, etc…) that are outside a Licensed Practical Nurse (LPN) scope of practice.
  • Provides in-home nursing visits on an as-needed basis.
  • Participates in administrative on-call rotation.
  • Enters OASIS assessments into the electronic medical record, as required.
  • Creates and/or approves Home Health Aide patient care plans.
  • Responsible for understanding patient outcome and HHCAHPS requirements and assisting with education of the staff as it relates to these regulatory requirements.
  • Reviews Telehealth data and identifies negative results.
  • Collaborates with patient, clinical staff and physicians when negative results or trends are identified.
  • Other duties as assigned.
