About The Position

Health Link Home Health and Hospice has been providing high-quality home health services throughout the San Francisco Bay Area and East Bay for nearly 20 years. We’re seeking a self-driven, compassionate RN Clinical Supervisor to join our dynamic team in Walnut Creek. At Health Link, collaboration, transparency, and teamwork are at the core of everything we do. This is an immediate opening for a motivated and organized nurse leader ready to make a difference in patient care. We are looking for someone willing to demonstrate exemplary performance in their ability to prioritize, multitask, organize, and learn quickly. We are looking for a detail-oriented individual, with an extremely pleasant demeanor who possesses excellent communication skills and is comfortable with handling a large call volume!

Requirements

• Active California RN License (Required)
• Home Health experience (Required)
• Excellent organizational, communication, and interpersonal skills
• Strong computer proficiency (Apple products & EMR systems)
• Ability to multitask and manage high call volumes with professionalism
• Associate Degree in Nursing (Required; BSN Preferred)

Nice To Haves

• 1–2 years of supervisory experience (Preferred)
• BLS Certification (Preferred)

Responsibilities

• Supervise and support Clinical Coordinators to ensure optimal patient care
• Oversee individualized patient care plans and ensure timely updates
• Address staff and patient inquiries, offering clinical guidance
• Triage patient calls and prioritize clinical concerns
• Implement and enforce home health clinical policies and procedures
• Ensure accurate, timely documentation and compliance with regulations
