The position requires a Bachelor's degree and experience in healthcare management, quality improvement, case management, performance improvement, or direct patient care. The coordinator will collaborate with multidisciplinary teams to develop and implement evidence-based clinical quality goals and outcomes, acting as a liaison and educator for these initiatives. They will also provide feedback on outcomes to medical staff and administration, mentor clinical outcome improvement teams, and create templates for streamlining team development and pathway implementation. The role involves participation in at least one nursing council, hospital, or medical center committee to improve patient care. The duties are subject to change by management.
