Coordinates the care of patients for a designated service throughout the continuum of care; serves as a clinical resource in area of specialty or expertise. Provides direct patient care including assessment, planning and evaluation of patients. Completes health history on patients and conveys findings to physician. Documents all patient interactions appropriately within EHR system. Provides diabetes education to patients, families, and caregivers. Analyzes blood sugar trends and recommends dose/technology changes. Develops and coordinates diabetes education material and programs. Provides phone consultation for prescription refills, sick calls, education and other patient concerns. Attends outpatient clinic sessions with providers to provide care, education and comprehensive assessment of patients. Coordinates patient care, orders diagnostic tests, convey findings to physician, assists in coordinating patient appointments and assists with referral to outside sources. Allocates resources, i.e., home health, project concern, and DME. Provides individual or small group education to initiate and coordinate diabetes care and diabetes equipment technology. Performs a variety of nursing administrative duties including compliance with environmental safety regulations, infection control standards, quality assurance, ADA standards and HIPAA regulations. Acts as a liaison and team member by facilitating communication and coordination of care between patients, families, and health team members. Educates patients and families about endocrinology related information. Develops, maintains and provides education resources for the non-diabetic endocrine patient. Other duties as assigned.
