About The Position
Full Time RN Clinical Manager for Hospital at Home Program. This role combines field-based patient care with leadership responsibilities, allowing you to make a meaningful impact in your community while guiding a team of skilled clinicians. You will lead and mentor home health staff and aides, oversee patient care coordination and program operations, and ensure clinical excellence and compliance with state and federal standards. Collaboration with referral partners, physicians, and interdisciplinary teams is key. Enjoy a consistent Monday to Friday schedule and a supportive team environment.
Requirements
- Active RN license in Florida, required
- Valid driver’s license and reliable transportation
- Strong communication, leadership, and time management skills
- EMR proficiency
Nice To Haves
- Minimum 1 year of home care experience, preferred
- Experience with Hospital at Home programs, highly preferred
- AlayaCare experience a plus
Responsibilities
- Manage patient intake, authorizations, and care planning for skilled and nonskilled services
- Conduct start of care and supervisory visits, as well as orientations and training for staff
- Provide clinical oversight aligned with best practices and agency policies
- Support and mentor field clinicians to ensure quality care delivery
- Maintain compliance with all regulatory and accreditation requirements
