About The Position
Preferred Home Care of Florida is seeking a Home Health - Clinical Manager. This is a growth opportunity leading to the potential Assistant Director of Nursing position.
Requirements
- Previous Home Health experience with Medicare, Joint Commission and PDGM training.
- Axxess System – Experience is a MUST!!!!
- Medicare Oasis proficiency within a Home Health Agency and QA experience
- Familiar with ICD-10 coding
- Ability to generate Plan of Care as per conditions of participation.
- Former Field Nurse in Home Health and/or Case Management experience.
- Excellent communication skills
- Knowledge of Medicare OASIS
- Knowledge of Medicare Discipline Frequency
- Computer Skills a must (EMR- AXXESS System Core)
- Knowledge of State, Medicare and Joint Commission guideline
- Must have a positive attitude and be a team player
- Three (3) years Home Health Nurse. Medicare OASIS, Quality Assurance (QA).
- Current Nursing License in the State of Florida
- Nursing degree from accredited school
Responsibilities
- Ability to audit OASIS for documentation and Discipline Visit notes
- Must make patient care a top priority
