About The Position

BAYADA Home Health Care is seeking a Registered Nurse Clinical Manager for our Assistive Care office in Elmsford, NY. In this role, you will help lead a compassionate team providing personalized in-home support that helps clients remain safe, comfortable, independent, and connected to their communities. Assistive Care is relationship-centered and focused on the whole person. Success in this role means building trust, understanding each client’s goals and preferences, supporting caregivers, and partnering with the office team to deliver dependable, high-quality service. The Clinical Manager is a hands-on leadership position that combines client advocacy, care planning, field supervision, staff development, quality oversight, and partnership with office leadership. You will serve as a clinical resource for caregivers and field employees, a trusted partner to clients and families, and an important member of the Elmsford office team. You will help ensure that services respect each client’s autonomy, dignity, preferences, and daily routines.

Requirements

  • Current Registered Nurse license in good standing in New York.
  • At least two years of professional RN experience.
  • Demonstrated ability to supervise, educate, mentor, and develop caregiving or clinical staff.
  • Strong assessment, care planning, documentation, organization, and follow-through skills.
  • Excellent communication and relationship-building skills with clients, families, caregivers, clinicians, office partners, and community resources.
  • Ability to protect client privacy and provide care that honors autonomy, dignity, choice, and cultural preferences.
  • Ability to manage competing priorities, make sound decisions, and adjust calmly as client and staff needs change.
  • Ability and willingness to travel throughout the service area for client visits, staff support, and office or community activities.

Nice To Haves

  • Home care, community-based nursing, long-term care, geriatric care, or supervisory experience preferred.
  • Experience supporting clients with activities of daily living, mobility needs, dementia, chronic conditions, or other long-term support needs is a plus.

Responsibilities

  • Lead, coach, mentor, and support caregivers and other field employees who provide in-home services.
  • Conduct client assessments, supervisory visits, monitoring visits, and follow-up visits in the home.
  • Create, review, and update individualized care plans based on client needs, preferences, routines, and goals.
  • Help ensure clients receive appropriate support with activities of daily living, safety, mobility, personal preferences, and other approved service needs.
  • Educate clients, families, caregivers, and field employees on safety, fall prevention, infection control, service expectations, and when to notify the office of a change in condition.
  • Communicate care plans, service updates, and client preferences clearly to caregivers, families, office staff, and other appropriate partners.
  • Provide timely coaching, feedback, recognition, and re-education to field employees.
  • Support orientation, skills validation, competency assessment, performance feedback, and ongoing development for new and tenured employees.
  • Partner with the Director, Client Services Manager, and office team to support staffing, service delivery, quality, compliance, and continuity of care.
  • Review documentation for accuracy, completeness, and timeliness, and ensure required follow-up is completed.
  • Respond to client, family, and employee concerns with empathy, urgency, and a solution-oriented approach.
  • Identify and escalate changes in client condition, unmet health or safety needs, incidents, service concerns, and other issues according to BAYADA policies and applicable requirements.
  • Coordinate with families, physicians, ancillary providers, community resources, referral sources, and other partners when additional support may benefit the client.
  • Help connect clients and families with appropriate resources, which may include adaptive equipment, transportation, therapy, hospice, elder care, or other community services.
  • Participate in hiring, onboarding, quality initiatives, client satisfaction efforts, office meetings, and community engagement.
  • Build positive relationships with clients, families, caregivers, referral sources, and colleagues while modeling The BAYADA Way: compassion, excellence, and reliability.
