Position Summary This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. It is expected that the employee demonstrate behavior consistent with our core values\: Integrity, Accountability, Best Practices, Compassion and Synergy. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities. To provide intermittent visits to palliative and the terminally ill patient and family, providing symptom management, and teaching to maximize quality of life. Coordinates the efforts of the team, including medical, nursing and psychosocial support to provide appropriate care as the patient transitions to the end of life.

Requirements

  • Medical/Surgical experience in an acute care setting.
  • Able to provide direct patient care activities, such as transferring and positioning adult patients
  • Effective problem-solving skills
  • Ability to relate to a variety of health professionals in different settings
  • Good interpersonal and communication skills
  • Other duties assigned
  • Current California RN license
  • BLS issued by the American Heart Association

Nice To Haves

  • Home Health, Oncology, and Hospice experience, preferred
  • BSN Preferred
  • Hospice and Palliative Care Certification preferred

Responsibilities

  • Assesses present and potential needs of patient and family
  • Triages telephone calls/visits per agency standards, while addressing every call in a timely manner.
  • Review Plan of Care with patient, family, physician, IDT and evaluate or modify as needed
  • Establish realistic goals of care to ensure continuity and quality of care
  • Complete all documentation in a concise, timely manner per agency standards
  • Manage symptoms and teach patient/family care management per agency standards
  • Be at work and be on time
  • Follow company policies, procedures and directives
  • Interact in a positive and constructive manner
  • Prioritize and multitask
