Clinical Coordinator opportunity for RN managing with CRRT program with Inpatient Dialysis. Full time, primarily day shift. The Clinical Coordinator uses advanced nursing processes in decision-making, patient assessment, data analysis, development and implementation of methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. provides direct and indirect patient care as needed; oversees the delivery of patient care; and provides day to day management of programs to coordinate patient care as designated by the department. coordinates the delivery of safe, high quality patient care; identifies areas for study; and participates in and/or supports research. The Clinical Coordinator Utilizes advanced nursing processes in clinical decision-making, patient assessment, and care coordination to improve quality, efficiency, and patient outcomes for the assigned population. Provides direct and indirect patient care as needed and oversees the delivery of care within the designated department. Coordinates interdisciplinary care plans, ensuring alignment with patient needs, clinical guidelines, and organizational goals. Performs patient assessments, facilitates referrals, and ensures appropriate placement and transitions across the continuum of care. Collaborates with physicians, nursing staff, social work, and ancillary services to ensure coordinated and effective patient care. Identifies and addresses barriers to care and implements strategies to improve patient outcomes, including length of stay and resource utilization. Develops, analyzes, and evaluates clinical and operational data to identify trends and support quality and performance improvement initiatives. Ensures compliance with regulatory and accreditation requirements, including The Joint Commission (TJC) standards. Provides education to patients, families, and staff to support understanding of care plans, treatment options, and post-discharge needs. Serves as a resource, mentor, and educator to staff and team members. Facilitates communication among patients, families, and care teams to ensure continuity and quality of care. Participates in interdisciplinary team meetings and contributes to care planning and progression discussions. Supports departmental and system-wide performance improvement and research initiatives. Maintains accurate and timely documentation in accordance with organizational and regulatory standards. Advocates for patients and families throughout the care experience.
