About The Position

The RN Clinical Coordinator supervises and/or provides direct patient care for the Adult Hospice Program. The position supervises all other members of the Interdisciplinary Group either directly or indirectly through their immediate manager. The Clinical Coordinator must be effective in explaining the hospice concept of care to patients, families, physicians, resource and referral sources and other community members.

Requirements

  • Must possess a valid RN license with the State of Nevada Board of Nursing
  • A minimum of 1 year of experience as an RN in a hospice/home health setting is required
  • CPR/First Aid certification is required
  • A minimum of one (1) year relevant hospice experience is required
  • Must successfully complete a background check pursuant to NRS 449.188 and NRS 179A.100 along with a pre-employment drug screen and physical, TB testing, previous employment, reference checking, and an MVR (may include verification of education)
  • Must be able to read, speak, write, and understand the English language
  • Excellent verbal and written communication skills are required
  • Must be able to handle frequent interruptions
  • Must be able to cope with emotionally charged situations encompassing patients, families, and other caregivers
  • Must be available for prescheduled on-call hours, weekends, and holidays
  • Must be available to cover shifts as needed and call coverage as needed for direct patient care needs
  • Must be able to prioritize tasks appropriately
  • Must be able to order medications, DME, and supplies using the hospice approved formulary and supply list
  • Must have reliable transportation, a valid driver’s license, and current auto insurance

Responsibilities

  • Provides direct leadership, management, and oversight of the organization's four key clinical service areas: Triage, Admissions, Clinical Education, and Certified Nursing Assistants (CNAs), as assigned
  • Follows an individualized Plan of Care established by the RN for each patient and covers patient visits and case management as needed
  • Participates in management on-call duties
  • Assists with hiring, counsels, and administers progressive discipline to assigned staff members as needed
  • Assists in developing plans of correction in response to surveys as needed
  • Evaluates and conducts performance evaluations of staff
  • Ensures proper staffing and scheduling for patients
  • Supervises and delegates duties to multidisciplinary clinical personnel
  • Assists with coordination of Interdisciplinary Team Meetings
  • Understands/provides basic principles of palliative care/pain management within the Scope of Practice
  • Participates in Performance Improvement activities as requested
  • Demonstrates reliability by making every attempt to report to work on time, providing proper notification for absence or tardiness, participating in required meetings
  • Demonstrates ability to solve problems and make decisions by assessing situations and considering alternatives, seeking guidance and direction as necessary, utilizing proper chain of command
  • Demonstrates effective time management skills by coordinating activities to achieve maximum productivity and efficiency; willingness to adjust or revise work schedules as requested
  • Stays current with all mandatory agency and regulatory requirements
  • Manages budgets, hires, and trains staff, and assists in developing and implementing policies and protocols
  • Completes assigned projects within specified time frames
  • Assists in creating and implementing policy/procedures
  • Attends scheduled staff meetings and/or in-service training
  • Participates and interacts in all complaint processes as directed
  • Ensures that all regulatory standards are met by team, including State, Federal, CHAP and other regulatory bodies
  • Implements a delegation of responsibility for the availability of clinical services on a twenty-four (24) hour basis
  • Adheres to patient rights, abuse reporting, and confidentiality policies
  • Fosters and maintains an effective working relationship between peers, professional staff, and other personnel
  • Manages clinical aspects of patient care
  • Assures patients are admitted under correct requirements for hospice service
  • Coordinates staff meetings and/or in-service training as needed
  • Manages confidential information
  • Adheres to the departmental dress code requirements related to personal grooming and attire
  • Maintains regular and consistent attendance
  • Assumes responsibility for ongoing personal development and continuing education
  • Other duties, as assigned
